This pamphlet is intended to summarize incidents that have occurred in chlorine production, chlorine packaging, sodium hypochlorite production, and user facilities pertaining to the chlor-alkali industry that have been provided to The Chlorine Institute (CI). No nitrogen trichloride incidents are included as these are available in CI Pamphlet 21 (3.1). The information presented is not new as it has previously been reported to CI members in Issue Team minutes, or in the Performance Indicator Reports which is provided on an annual basis. In most cases the information originally presented has been reformatted and additional information such as lessons learned have been added to the report. The Chlorine Institute believes that by publishing information pertaining to such incidents in a single publication, such information will be more accessible to personnel in facilities producing, using, or otherwise handling chlor-alkali chemicals. The incidents included in this pamphlet do not represent all the incidents reported to CI. Those selected for inclusion are believed to be of interest to the user of the pamphlet and are based on information provided to the Health, Environment, Safety and Security Issue Team (HESS) and its predecessors. This pamphlet does not include transportation incidents. Refer to the CI Transportation Incident Report for details on transportation incidents (3.1).

After each description of an incident, a “lessons learned” section follows. Where applicable, this section refers to a CI pamphlet number where an applicable CI recommendation is contained. The reference section provides the complete title of each CI pamphlet so referenced.
